Curry Scorecard

Our application for local chapter/member awards is named after Keys Curry, Texas ’54, a longtime volunteer to the service of the chapter and former head of the Texas Alpha Endowment Fund. ┬áThis “Scorecard” is now used as an application for all member awards.

Please complete the information below for the previous calendar year (January – December). Fields with an asterisk (*) are required. Enter “None” where you have no responses to those questions.

You will be asked to upload confirmation of your GPA. You may scan an official copy of your transcript or grab a screen shot of your student record from online. Please have that ready. You also are offered the opportunity to upload a copy of your most recent resume. It’s not required, but helps us in our evaluations.

The Scorecard covers a lot of information, so you may wish to print a copy of the form first and create your responses offline, then come back and enter everything in at one sitting. Once you submit the form, it will go to the Scholarship Committee for their review. Their decision on awards and scholarships will be final.
